New Patients

Please ask at the reception desk where you will be required to complete a registration form and a brief questionnaire.

You are also required to provide 2 forms of identification – 1 photographic (passport or driving licence) and 1 utility bill as proof of address.  

You will then be invited to book an appointment with the Practice Nurse or Healthcare Assistant for a New Patient Health Check.

When you have completed all of the details, please bring the form personally to surgery along with your proof of identification.

You will not be registered with us until you attend so that we can get a signature from you as confirmation that you are who you say you are.

When you register you will also be asked to fill out a medical questionnaire, this is because it can take a considerable time for us to receive your medical records.

There is an online version of this form too (below), which you may fill out and send to surgery.
